While it is not necessary for you to share a copy of your will or estate documents with New Leaf Behavioral Health both you and New Leaf Behavioral Health benefit when you document your intent for making a planned gift.

Sample Bequest Language
The following provides sample language for making a bequest, conveyed through your will, in support of New Leaf Behavioral Health.
I give, devise and bequeath___________percent (___%) of my residuary estate [or, the sum of _________dollars ($____)] to New Leaf Behavioral Health, located in Raleigh, North Carolina. New Leaf Behavioral Health shall treat this bequest as part of its endowment, and the income paid out of the Fund shall be used for the benefit of the New Leaf Behavioral Health in any matter in which the Director of the New Leaf Behavioral Health deems appropriate.
I give, devise and bequeath___________percent (___%) of my residuary estate [or, the sum of _________dollars ($____)] to New Leaf Behavioral Health, located in Raleigh, North Carolina. These funds shall be expended for the benefit of the New Leaf Behavioral Health, in any matter in which the Director of the New Leaf Behavioral Health, located in Raleigh, North Carolina deems appropriate.
Also, we request that you include the following paragraph:
If, in the opinion of the Board of Directors of the New Leaf Behavioral Health, all or part of the gift cannot appropriately be used in the manner herein described, the Board many use the gift for other purposes as nearly aligned to my original intent as the Board deems appropriate under the circumstances.
Transferring appreciated securities is an excellent way to support New Leaf Behavioral Health and receive tax benefits.
Transferring Securities Held Electronically
Your bank or broker may transfer stock to New Leaf Behavioral Health’s brokerage account.
Form to transfer Stock
Please confirm the transfer with our office by email: firstname.lastname@example.org. Often, shares are transferred into our account without a donor’s name. The email will enable us to match your name with the correct stock gift and ensure the funds are designated to New Leaf Behavioral Health, located in Raleigh, North Carolina.
Transferring Securities in Certificate Form
Please send by first-class mail or hand deliver the unendorsed certificate(s) along with a letter of transmittal which includes your name, address. Send to:

In a separate envelope, mail to the same address a copy of the transmittal letter along with a stock power executed in blank for each stock certificate you are donating. You should sign one stock power for each stock certificate and have your signature guaranteed by a bank officer. The bank officer should not date the signature guarantee. For most expedient handling, do not fill in the New Leaf Behavioral Health’s name as transferee on either the stock certificate or the stock power and do not send the stock certificates to a transfer agent for transfer into the New Leaf Behavioral Health’s name.
If your certificate represents more shares than you wish to donate, you must provide your social security number and a letter detailing how many shares you wish to donate and how many shares should be re-issued in your name. It generally takes four to six weeks to receive your new certificate.
If you prefer to have your certificate re-registered, the shares that you wish to donate to New Leaf Behavioral Health should be registered to “New Leaf Behavioral Health.” The tax identification number is 32-0204218.
Many employers sponsor matching gift programs and will match charitable contributions made by their employees. Please choose New Leaf Behavioral Health when selecting on your matching gift site.
